Below are the links to the application. Please note that all children must be up to date on their childhood vaccinations and shot records must be provided.
Non-vaccinated children will not be accepted. Your child does NOT need to have the covid vaccine.
Here is some basic info before you fill it out.
Food
Snacks, meals, milk and water are provided for the children.
Diapers & Potty Training
If your child is wearing diapers, please bring diapers for diaper changes.
I ask that you bring more than one days worth of diapers, so you don't have to bring them daily.
If your child has accidents, they will need to be wearing a diaper/pull-up and not panties/underwear.
All children must have one change of clothes kept here for accidents or in case needed.
Payment
Payments are due the Friday before the week of care. If not paid before, there will be no care on Monday. We accept DHS, cash, check and FB Pay. We also accept credit card payments using Procare. Please note that parents are responsible for the transaction fee (usually around $3-8).
You can use the above form for the child's Immunization record or you can request a print-out from the child's doctor.
Please print out the Enrollment Packet, fill it out and get it back to me. Before enrollment, I will also need the last page of the Parent Handbook signed and returned. You can either email the scanned application to LPChildCare@yahoo.com or give the application to me in person. It's recommended to bring the full Enrollment Packet, filled out, to the interview appointment. Please keep in mind that before your child can start here, I must have the Enrollment Packet completely filled out with everything signed, your child's up to date shot record, and you must also bring the final page of the Parent Handbook signed.
